I appreciate the trust you have shown in allowing me to provide for your medical needs.

At this time I will be retiring from practicing clinical medicine.

My practice will close for patient appointments on May 26, 2015. No medical services will be provided after that date. If you have surgery scheduled after May 8th, 2014, we will make every opportunity to schedule your surgery prior to this date. This will allow me to see you for your post-operative care.

Many of you have met my esteemed colleague, Dr. Jonathan Song of TLC Medical Group. He is a board-certified OB/GYN who specializes in women’s health, gynecology and minimally invasive surgery. I have worked with Dr. Song for over 13 years, and he has covered my practice when I have been away from the office. Dr Song has agreed to be the custodian of the medical records from Renaissance Women’s Health. He will also provide follow up care to the patients of Renaissance Women’s Health if you should choose. If you should need your records after May 26, 2015, please contact his office for a copy of your records. They can be reached at 630.513.9160. He will also be happy to take care of your gynecologic needs.  Should you choose to have care elsewhere, you may want to contact the county medical society, a local hospital or your health care plan for a referral.

If you want another physician to have a copy of your medical records, please fill out the authorization form on the back and return it to my office no later than May 26, 2015 or to TLC Medical Group (630.513.9160) after that date. There may be a nominal charge to obtain your records.

If you need medical care prior to the closure of my practice listed above, please do not hesitate to contact my office or the Emergency Department at Delnor Hospital at 630.208.3000. My desire is that my retirement be as easy on you as possible.

I have known many of you for several years, and appreciate the trust that you have placed in me to provide your medical care. The decision to retire was not an easy one, and I am grateful for the opportunity to have been your physician.
